We are seeking a VP, Skilled Home Health and Hospice Operations to lead our skilled services department. The ideal candidate will possess a strong background in healthcare management, strategic planning, and clinical expertise in home health care and/or hospice, including forming and managing interdisciplinary teams.  Success in this role depends on strong leadership, collaboration, team management, innovation, and a relentless commitment to clinical excellence and patient safety.

Responsibilities

What You Will Do:

  • Develop and execute strategic plans to support consistent growth, clinical quality and operational execution in skilled home health and hospice care agencies. Provide strategic leadership to the design, implementation, and measurement of coordinated care model.
  • Operational Oversight: Provide leadership and direction to the skilled home health care/hospice management team(s), ensuring efficient operations, adherence to regulations, and high-quality service delivery. Demonstrated expertise in strategic planning, operations management, and clinical quality improvement. Establish key performance indicators (KPIs) and metrics to assess the effectiveness and impact of skilled home health care services, regularly evaluating and reporting on performance to executive leadership.
  • Clinical Care Standards: Maintain and enhance clinical standards, protocols, and best practices to ensure quality care for clients. Stay current on emerging trends, research findings, and innovations in home health care, exploring opportunities to integrate evidence-based practices and novel approaches into our service delivery model.
  • Team Development: Recruit, mentor, and develop a high-performing team of skilled home health and hospice care agency management, fostering a culture of continuous learning and development. Provide leadership, guidance, and support to promote teamwork, collaboration, and professional growth.
  • Embrace the critical importance of effective collaboration and seamless integration with other service lines to achieve organizational goals and ensure high-quality outcomes.
  • Quality Assurance: Implement quality assurance programs and processes to monitor and improve the effectiveness and outcomes of skilled home health care services. Develop and implement ongoing education and training programs for skilled home health care staff, keeping them abreast of best practices, innovative technologies, and regulatory changes.
  • Compliance and Regulatory Management: Stay abreast of relevant regulations and ensure compliance with all legal, regulatory, and accreditation requirements governing home health care services. Provide education and training to clinical staff on documentation standards, coding requirements, and regulatory updates.
  • Financial Management: Collaborate with finance and other stakeholders to develop and manage budgets, optimize resource utilization, and achieve financial goals.
  • Partnership Development: Cultivate and maintain strategic partnerships with healthcare providers, payers, community organizations, and other stakeholders to expand and enhance our skilled home health care offerings.
  • Technology Integration: Identify and leverage innovative technologies to improve service delivery, communication, and efficiency in skilled home health care operations.
  • All other duties as assigned.                                                                                

Qualifications

Qualifications:

  • Bachelor’s degree in nursing, Healthcare Administration, Business Administration, or related field required; Master’s degree preferred.
  • RN license preferred.
  • 8+ years of clinical experience in home health care or hospice with 3+ years in multi-site, operations leadership.
  • Strong leadership abilities with a focus enabling consistent growth, clinical quality and operational execution in skilled home health and hospice care agencies.
  • In-depth knowledge of clinical practice guidelines, regulations, and reimbursement policies.
  • Experience in participation, forming, and developing interdisciplinary teams.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to collaborate effectively with interdisciplinary teams and external stakeholders.
  • Commitment to continuous learning, professional development, and evidence-based practice.
  • Strong understanding of healthcare regulations, accreditation standards, and compliance requirements related to home health care services.
  • Excellent interpersonal, communication, and collaboration skills, with the ability to effectively lead and motivate a diverse team.
  • Demonstrated success driving performance, achieving financial targets and fostering innovation in care delivery.
  • Ability to adapt to changing environments, prioritize competing demands, and make sound decisions.
